Social Work Job Description
Professional License Requirements: MSW or BSW
State License Requirements: Not a set requirement for general practice (unless specified by employer)
- Meets with patient and their family to assess and scope of assistance needed as it relates to health, financial, social and emotional needs
- Consults with facility leadership and colleagues to formulate a crisis intervention plan
- Presents crisis plan to patient and their family
- Monitors and records patient's progress on a consistent, scheduled basis and as required by the hiring facility
- Assists in planning patient’s release from program, including post-treatment coping skills and education
